Jordan graduated from Keene State college with a bachelor’s degree in health science in 2006. In 2018 he began his career in health care as a CNA at Candlewood Valley. Not before long he made his way into the position of Concierge Director where he welcomes patients and helps to coordinate their care and enrich their daily lives.
Jordan’s passion lies in fostering good relationships with residents through positive experiences. He is gifted at creating healing environments that encourage individualized care and personal confidence.

I cannot say enough about Candlewood Valley Health. The staff really care and are quick to respond to one’s needs.
The Physical therapy staff are wonderful and take great interest in your progress. The food presentation is always delivered with a caring attitude. Each meal I have been given has been extra tasty. The kitchen does everything they can to please the taste buds of all of us here.
Jordan always seems to be there to help in anyway he can! The Aides are very qualified and helpful. There are several that I have come quite attached too. On a scale of 1-10, 10 being the best I would rate Candlewood Valley a 10! A score that is earned and well deserved!
Last but not least thank you to nursing for your outstanding care! All in all, staff here is helpful and kind from my arrival all the way up to my discharge.
Gratefully,

Thank you for your wonderful care of Oma!
My wife and I had a trip planned to go to the Holy Land but we had an issue. Who would take care of my Mother in Law, Oma? We would be gone for ten days and we wouldn’t be available if an emergency occurred.
Over the previous 6 months or so I had been hearing wonderful things about Candlewood Valley from a variety of people. I reached out and arranged for Oma to stay as a respite guest while we were away.
I must admit that we travelled on drop off day to New Milford with great fear and trepidation as to how the stay would turn out. We had a dear friend meet us at Candlewood to ease any fears this 94-year-old might have.
Long story short, Oma did excellent during her respite stay, receiving wonderful care. We had a number of friends and family drop in on her and each of them found her content and well cared for.
We are already booking her next stay!

‘Thanks’ to all who ‘put me back on my feet’
To the Editor:
I would like to express my thanks and gratitude to all the organizations and people that took part in my rescue and recovery from a fall on Oct. 19.
I broke my right thigh femur.
The Bridgewater ambulance medical team transported me to New Milford Hospital with expertise and care.
Dr. Edmund Ganal of New Milford Orthopedic repaired the damage with my thigh so that I would walk again.
I say thank you.
I spent three weeks at Candlewood Valley Health & Rehabilitation Center in New Milford under the director David Segal and his excellent staff of nurses, aides, physical and occupational therapists who put me back on my feet and walking.
They made my stay there a very positive one and I am very grateful to all.

To Whom It May Concern,
I would like to inform you of the great care I received in your facility during my recent stay, especially the nursing staff and the rehabilitation staff.
One standout in the nursing staff was Crystal Dube. All were very good, but Crystal gave me superior care. Always ready to answer my questions and provide any help I asked for.
Dr. Jen Tuss was very helpful with my short rehab there. Very knowledgeable and always cheerful with great advice for my recovery. She took great care to be sure I was discharged with all the information I needed for a complete rehabilitation.
Everyone was very helpful and kind…..from admission to discharge. Thank you.

Dear Elaine and Committee:
On behalf of Resident Council and Candlewood Valley Health & Rehabilitation Center, far which I am President. I would like to thank you and your committer for a job well done. All the events planned for Customer Service Month in August were well organized, planned down to the smallest detail. The residents and families enjoyed everything.
For the picnic the DJ was great. It was real a pleasure to see children enjoying our picnic under the tent. We all loved watching them do gymnastics and spending time with their loved ones.
Of Course a good game of Bingo under the tent was a lot of fun also and then we are going to have a campfire tonight. Thank you all for all the hard work you put into each and every event, it really showed and was much appreciated.
Sincerely Yours,
David T. Resident Council President

We can’t thank everyone enough at Candlewood Valley. My husband, John, came to you after extensive back surgery and numerous complications. He was prepared for hard work, and he was so pleased that everyone in the Therapy Department helped him to reach his potential and return home after a total of three months. Dr. Den Tus and her staff were amazing. A special thanks to Chandra and Kathleen for their expertise and encouragement. Everyone made his time there so comfortable with their smiles, assistance, and friendly attitudes. The nurses and aides were so wonderful. I think what was most impressive was the team effort. Everyone worked together and helped each other to make the patients comfortable and successful.
We look forward to your reunion and reuniting with all your great staff.

Dear Mr. Segal,
On behalf of my family, l’d like to thank the entire staff at Candlewood Valley for the care they provided my mother, Jane During her stay in the Dogwood unit, she was treated with kindness and respect, and every effort was made to keep her physically well and engaged with her surroundings. Although dementia took many things from my mother, your staff made sure she never lost her dignity.
When Jane was in her final days, my sister and I stayed with her in her room Christmas Eve through the night until she passed on Christmas Day at about 4:30 pm. Every member of the Dogwood staff made us feel welcome, supported us, expressed their sympathy, and grieved with us at the end. Their advice and reassurances were of great help to us, as were the small things like cold drinks and comfortable chairs. The maintenance staff were so discreet and understanding of our situation, and the kitchen staff were generous with snacks, sandwiches, drinks, and even a lovely Christmas dinner.
The nursing staff in Dogwood was outstanding in every way. Sarah, Geoff, and Kathy during the day were a constant source of help, information and understanding. On Christmas Eve, Renier kept a close watch on Mom’s comfort throughout a long night. And Jackie was there with us at the very end, encouraging us, and ensuring that Jane’s passing was as peaceful as it could be.
The aides who cared for Mom’s every need while she was with them have our eternal gratitude. They were unfailingly kind as they did for her what she could no longer do for herself. We thank Pam, and all the other aides, many of whom we may never have even met, for the attention, the hugs, and the fabulous hairstyles they gave Jane while she was in their care.
We are also very grateful for Cindy’s help during Mom’s stay. She was a great help with the paperwork as well as the personal. And the staff behind the front desk, and the desk in Dogwood, were always kind and welcoming. We appreciate all of you, and thank you all.
Sincerely,
Ann Marie

Praises to the Wonderful Care at Candleused
Health & Rehabilitation
As a recent month-long patient at Candlewsed Valley Health & Rehabilitation
Center, due to foot surgery leaving me immobile, l hadn’t anticipated what a
pleasant experience it would be there. My care was extraordinary, from the cheery Good Morning “to a soft Good Night.” There was expert care of my wound; daily physical therapy; nutritious, yet delicious, meals in super-clean rooms; and special arrangements for my friends who came to play Scrabble or Bridge. All this was accomplished by a wonderful staff of nurses and aides happily working together. Candlewood Valley Rehab rightfully deserves a national Five-Star Rating. New Milford is certainly fortunate to have this terrific facility right here. Add another star from me.

Thanks for Care of Mom
I felt the need to write this letter to let the community of New Milford know about the outstanding care my mom. Joan Drake, received as a resident of Candlewsed Valley Health and Rehabilitation. She lived there for about three and a half years before we recently moved her to Virginia with us.
During this time. Mom had dementia and was more recently diagnosed with endometrial cancer, neither of which is easy for someone to care
for. But all the nurses and CNAs on the Elen Tree wing did so with grace and respect toward my Mom.
She went through five weeks of radiation at New Milford Hospital, in which she had to go every day for a 10-minute treatment. Everyone involved with caring for her at both facilities, including Danbury Ambulance, were wonderful.
This all happened right smack in the middle of winter in February. Not fun! The whole staff at Candlewsed treated my Mom with
patience, respect, dignity, privacy, and compassion. I always had a sense of security knowing that she was safe, as happy as can be expected and cared for. It was a huge comfort. Since writing this, sadly, my Mom has passed away; she is no longer suffering.

My husband, Robert Curtis, was a resident of Candlewood Valley Health & Rehabilitation facility in New Tifford for the past one and a half years.
My family and I were so impressed with the staff that cared for him with such care and compassion. The nurses, aides, doctors, physical therapists, speech pathologists, recreation staff, social workers, and hospitality staff all treated him so well and made us feel welcomed when we visited him.
Starting at the reception area, the welcome we received was obvious throughout the facility. The facility has truly earned its Five-Star status.
Thank you to all who helped make my husband’s stay so home-like.

Mr. David Segal
Administrator
Candlewood Valley Health and Rehabilitation Center
30 Park Lane East
New Milford, Connecticut 06776
Dear Mr. Segal,
Our dear was a resident of Candlewood Valley Health and Rehabilitation Center’s
Dogwood unit from July 2019 to March 2020. We write to memorialize how much we valued the amazing care she received during her relatively short time at the Center, and how much we appreciate the truly wonderful staff that work so hard to provide kindness, care, and a sense of dignity to the residents.
From the start, we felt comfortable that her care would be a collaboration. ( greeted us
during our first visit, listened to our concerns and apprehension, and explained that she and her staff would work with us to make sure that would make a smooth transition. From her first day, the staff was attentive, caring, and willing to work with us on our various (and, admittedly numerous, requests), everything from the type of bed and wheelchair she had, to her physical therapy, to her wound care. I visited nearly every day and called twice daily, and each time, the staff made time to check in with me, update me, and answer any questions. And they did all of this with a smile. They treated me and their residents with patience and respect.
Particularly appreciate the opportunity I had to visit I on three occasions after the
COVID restrictions, and before she passed away. I also appreciate the comfort and care that the staff provided to her in last days. Going through Dementia with a loved one can be a terrible process, but we felt comfortable trusting the staff to take care of her, and felt great relief that she was well-cared for.
We extend our thanks to all the administrators, nurses, staff, and volunteers at the Center for all of their great and tireless work, especially during this challenging time of COVID-19, where they put themselves at great risk to care for our loved ones.

I just wanted to inform you of the wonderful stay I experience at Candlewood Valley.
On the night I arrived, I was greeted with a smile from Christopher (male nurse on my ward) and 2 other employees. I was wheeled into a private room that I was impressed at first site (at first I thought they had the wrong room!) The warm greeting I received made me feel instantly calm.
The PT person who took excellent care or me called Dan, welcomed me the next day when I got up. A few times I told Dan I was not up the PT that day, and within mins I was on my way to my daily exercises. One day I looked at Dan as we walking down the hall, “how do you do that, whenever I say I am not up to PT you change my mind, every time!” He smile and said “the power of persuasion”.
Every member of the nursing staff were “outstanding”. All hard workers and caring. I want to also state “Jenny (a nurses aid) was a love! When she helped me take a shower she was singing! I loved that, little things make a patient feel better that she may not even know! Please tell her to keep on singing Jenny, and let her know she is in the right job! She was a pleasure to see every day she worked.
Also the male nurse Chris was another exceptional employee, always making me laugh, and holding my hand when I was in pain late at night, that was when my pain was the worst. Other staff members I do not remember their names, but were also “outstanding, and I will never forget them”. I would recommend your center to anyone who needs care after a knee surgery.
Thank-you for all you do, and keep up the excellent work, it does not go unnoticed!!

I hope this thank you finds the staff at Candlewood valley rehab well. before I go further, I want to give my apologies for not remembering the names of many of the people that helped me through such a difficult period in my life.
So many staff members helped me in so many ways. the nurses, therapists, and especially the CNAs who were invaluable on a minute-by-minute basis. I can see you all clearly in my memory, but unfortunately, I can’t place all the names.
When I arrived at Candlewood, I could barely move myself into bed, much less take care of my simplest personal needs. both of my legs were broken, my right arm and wrist held together by an external metal bar, my neck and many ribs were broken too. Simply put, I was in need of constant and fairly intense care, and all of you provided it. i slowly was able to do things again; from using the bathroom, to taking a shower, and yes, even walking.
Also, I need to thank some of the other folks, including the gentleman who was the handyman. Due to the horrible winter, he was constantly removing snow. When I arrived, I needed a special bed. He put it together in minutes. Thank you to the housekeeping staff who kept our surroundings meticulously clean and safe.
I was in the short-term rehab unit, this is important to mention because of all the un-named folks, but also another special moment. A nurse from the senior wing. Who was working a double-shift during one of the storms, went out of her way to give me a bath in bed. I mentioned that I felt dirty, and that was all it took. She could have done less on such a busy day, but instead took the time to make me feel better. This sums up my experience at Candlewood: people who gave 110% to make me better.
I thank you all most sincerely from the bottom of my heart. You are all the best! I love you all. Thanks!

It takes a village…
On Saturday, nov. 21, my dad arrived via transport and was gurneyed into room B8. Thirty-five days later, he determinedly walked from his room, out the front door, to my car.
I had the pleasure to meet most of the team that made this possible.
His nurses… Suzanne, Nancy, Andrea and Chris whose professionalism, medical knowledge, and gentle bedside manners helped him heal.
His aides and helping hands… Johanna, Phil, Kevin. Donna, Rebecca Lynn, Heidi, Pam, and Ronnie. Your smiles and positive outlooks are unmatched. You were as important as any medicine in my dad’s recovery. I was blown away by how respectful you all were to my dad and to other patients that due to illness and injury could have had their dignity compromised. Your respectfulness and good natures preserved their dignity, keeping their focus on healing.
The physical therapy team… Jen, Mary Elizabeth. Elise and Nina. Although he complained that therapy was unrelenting and sometimes a bit early in the morning, secretly, he enjoyed it. No doubt he will miss it.
Recreation… Theresa, thank you for filling the halls and dining room with live entertainment and with the spirit of Christmas. Also, thank you for getting my dad to play bingo. His first bingo win was a turning point in getting him out of his room and regaining a positive outlook.
The kitchen and dining room staff… Laurie, Doug. Casste, Fryan, and many others. Thanks for being so gracious and for serving smiles with all the meals. The corned beef and cabbage dinner was hailed by my dad and Mr. P as a “real treat.” It was the first time in weeks that my dad ate everything on his plate.
Amy… your kindness and personal connection with my dad via your grandmother helped my dad on many fronts. Your patience with a patient who daily planned his “escape” was extraordinary.
Elaine, Georgianna, and Katie… thank you for orchestrating the long-overdue haircut just in time for my dad’s off-site holiday party. He looked great and was uplifted by the many compliments he received. And others I may have missed… Laura, the dietician, the receptionists, Deb, George Inn, and the relief nurses. The night staff, the cleaning crew, the behind-the-scenes team… thank you, too, for your contributions to my dad’s well-being. I know many of you lifted his spirits with your daily conversations.
And lastly, a special thanks to Ann, the administrator, who undoubtedly leads this awesome team. I thank you all for doing what you do, for doing it so well, and each of you for your hand and heart in my dad’s recovery.
