What are the five-star quality ratings?

Nursing home ratings come from:

  • Health Inspections
  • Staffing
  • Quality Measures

A star rating is provided for each of these three sources, in case some areas are more important to you than others. Then, these three ratings are combined to calculate an overall rating.

Why is this important?
Nursing homes vary in the quality of care and services they provide to their residents. Reviewing health inspection results, staffing data, and quality measure data are three important ways to measure nursing home quality. This information gives you a “snap shot” of the care individual nursing homes give.

Use the Five-Star together with other information
The Five Star Quality Rating System is not a substitute for visiting the nursing home. This system can give you important information, help you compare nursing homes by topics you consider most important, and help you think of questions to ask when you visit the nursing home. Use the Five-Star ratings together with other sources of information.

Candlewood Valley Health and Rehabilitation Center is a 148 bed skilled nursing and rehabilitation facility situated in New Milford, Connecticut, right in the heart of Litchfield County. We are located in close proximity to both New Milford and Danbury Hospitals. In addition, we are just a short distance from the New York border of Dutchess and Putnam counties.

Candlewood has been providing award winning care in the Litchfield County area for more than 40 years. We offer a broad range of health care services, including, but not limited to: Post Acute Care, Short Term Inpatient/Outpatient Rehabilitation and Long Term Skilled Nursing. Candlewood also has a 42 bed secure unit dedicated to supporting those affected by Alzheimer’s disease and other cognitive conditions.

Our facility is situated on a bucolic piece of property, nestled in a residential neighborhood, yet conveniently only seconds from Route 202, which runs throughout Litchfield County. Our park-like grounds provide a lovely setting for visits with family and friends.

We want you to be confident that you have selected the best facility for your health care needs. When possible, we encourage you to meet our team and tour our facility in advance of a rehab or long term stay. We understand this is not always possible when unexpected illness or injury occurs. You can rest assured that our team is committed to providing you and your loved ones with exceptional health care in a warm and supportive environment.
